Consumer Directed Services (CDS) is a Medicaid based program in which persons with illness, aging needs, or disabilities requiring assistance with daily living are allowed to live independently within their own home and communities.
Consumer Directed Services include: Personal Care Assistance includes but is not limited to:
- Personal Care- Bathing, grooming dressing, personal hygiene
- Toileting- Ostomy or catheter hygiene, bowel and/or bladder routine, general toileting activities
- Health- Use of transfer device/mobility issues/prostheses, passive range of motion, manual assistance with medications, treatments, cleaning and maintenance of equipment
- Housekeeping- Cleaning, dusting, bed linens, laundry, trash
- Meals- Meal preparation and/or assistance with eating, washing dishes
- Transportation- Essential shopping/errands, medical appointments, school or employment
Independent Living Waiver Services which include:
- Case management
- Environmental Accessibility Adaptations- adaptations to participant’s home to ensure the health and safety of the individual and/or enable the individual to function with greater independence in the home. Adaptations include, but are not limited to ramps, grab-bars, widening doorways, etc
- Specialized Medical Equipment/Supplies
A Caring Heart, LLC provides advocacy and support for clients in the CDS program. Being a participant in this program gives persons with a disability control over their own life. Participants in CDS may choose to have care provided by family members over age 18 with the exception of a spouse or seek to hire a qualified personal care attendant. Personal Care Attendants (PCA) must pass a criminal background check through the Missouri State Family Care Safety Registry. The PCA’s job description can include: housekeeping, laundry, cooking, personal care and transportation. A Caring Heart, LLC will assist the consumer with managing every step in the selection of a Personal Care Attendant (PCA). A member of A Caring Heart, LLC‘s management team is required to make frequent phone contact and home visits to insure your plan is meeting your needs and that program rules and regulations are being followed. A Caring Heart, LLC will train you ~ the consumer, on your responsibilities as well as our responsibility to you.
